The second secret medal is called pervert. During the battle scene, click on something that would make you a pervert if you touched it in real life.
I made a walkthough video about this game and It has all the metals including the secret metal ones.

you don't have to spam it you can just hold it and the 4 th time you jump the fire I always die even when i press up
